Fremantle Family Crisis Accommodation and Referral Service is a 12 month pilot project between DCPFS (Rockingham and Fremantle) and St Patrick’s Community Support Centre.
The program provides short term (12 weeks) accommodation to families that are in housing crisis.
St Patrick’s has supplied two two bedroom houses that can house a family of up to 2 adults and 4 children. The houses are in Hamiliton Hill and are fully furnished including bed and bathroom linen and kitchen appliances and utensils.
Clients are referred by agencies that have an Memorandum of Understanding and Case Management of the client is provided by both agencies.
The FFCARS Support Worker gives assistance to develop a Case Management Plan that identifies the goals around housing and ongoing support needs.
Clients are case managed for the 12 weeks and assistance is given to link into services and meet identified goals for housing.
