St. Patrick’s Community Support Centre

The supported case managed accommodation service is provided to males over the age of 18 who are homeless or at risk of homelessness. The program requires participation in case management on a weekly basis and other activities including breakfast club meetings on a daily basis. The program runs for 12 weeks and is designed to assist clients to access services and supports both internal and external to result in medium to long term accommodation.

Eligibility:

  • Be in receipt of full Centrelink benefits
  • Possess current health care card
  • Be able to live independently
  • Be willing to participate in case management and breakfast club activities
  • Agree to abide by the house rules in particular dry house rules
  • Meet any additional eligibility criteria as required

Face-to Face Assessment interview:

  • Your suitability for housing and the current availability of accommodation
  • Any specific requirements and support needs
  • Your income and what the costs of entering into accommodation will be
  • Any other issues that may impact your ability to maintain tenancy and how these can be addressed

Program suitability:

Supported accommodation is designed to assist clients with an immediate response to their current situation and provide accommodation as soon as possible when  all eligibility criteria is met. However, at times there may be factors that even in crisis may not result in immediate housing. Support staff will do their best in the face-to-face assessment interview to ensure they work through these barriers with you and refer on to appropriate services if applicable.
