Street to Home
Street to Home is an interagency, integrated support service for people experiencing rough sleeping.
St Pat’s Street to Home team of specialist, assertive outreach workers engage with people in the community who are sleeping rough, couch surfing or at risk of homelessness. Based in Fremantle, they assist people in accessing available services and work with them to identify and address the often complex, personal and systemic barriers preventing them from accessing housing and appropriate supports.
